Change of instructions for use:
Following feedback from users of our on-line forms we have made some slight changes to the system.
The system used to timeout if you had not completed the questionnaire. The questionnaire is now set to timeout after 3 hours but should you wish to save the form you can submit it and you will receive a copy via email. To go back in and edit the form click on the same link you were supplied with by the system and insert the user code. Enter JUST your email address on the next page and press enter. This will bring back up the form for you to complete.

H&G Healthcare have produced a new-employee questionnaire specifically for the healthcare sector. This questionnaire complied with the requirements of the NHS for contract and temporary workers working in both the NHS and private medical sector. Occupational Health Referral form
Use this form for a referral for long-term sickness or absence management or any other cases where you wish to refer an employee for a medical. You will need to complete the form and upload a job description.

Once you have completed your questionnaire please press the submit button and it will be encrypted and sent to our occupational health team who will review the questionnaire and then contact you with the outcome.
